Service Form Questions

Solution
  1. If both clients participated in the session should I enter one session form, for a duration of 60 minutes.

  2. Is a needs assessment required before entering any other service forms?

  3. If a client comes in repeatedly over the course of a month and covers more than one topic (e.g. housing, money and finances, employment, etc.) how should this information be tracked?

  4. If a baby is in an Information and Orientation session with its mother, but does not directly participate in the session, is that a family orientation?


  1. If both clients participated in the session should I enter one session form for a duration of 60 minutes.

You would start by ensuring both clients have been added to OCMS by checking find a client. Once both clients are entered in the system you will open one session form with a time of 60 mins. Then add both clients and create (2) two service forms, record one for the husband and one for the wife, each record would have all topics checked off because each participant heard the information provided to the other.

 


  1. Is a needs assessment required before entering any other service forms?

No.

It is important to note that NARS is only to record a global needs assessment. For the most part, NARS will be for new clients the first time they come in for a service. A new assessment could be done whenever the client's situation changes (e.g. they lose their job, have a baby, etc.).

If you do not conduct a global needs assessment, there is no need to create one.


  1. If a client comes in repeatedly over the course of a month and covers more than one topic (e.g. housing, money and finances, employment, etc.) how should this information be tracked?

All these subjects are located in Information and Orientation form, there is the ability to check off the additional topics covered.

However, if a new service is being provided  on a new day a new record should be created. If it is an extension of an existing service, the existing record can be edited.


  1. If a baby is in an Information and Orientation session with its mother, but does not directly participate in the session, is that a family orientation?

Only count clients who participated in the session. In this case, the baby did not participate in the session and therefore it is a one-on-one session and the baby does not receive a record.
