Employment Group Activity Attendee Form - Named Clients

Learn how to complete the  Employment Attendee Forms for a Group Activity with Named Clients.

Employment Group form and Attendee forms order

Due to the complexity of these forms, they must be completed in a specific order to ensure that your reporting will be correct.

  1. Group Form - This form must be completed first to ensure correct settings in the attendee form

  2. Attendee Form for one participant

  3. Copy to all others

  4. All Attendee settings must be completed
  5. Make changes if needed

NOTE: for all attendee setting to work properly on any session type, the main form needs to be completed so the partner can be picked up.

After you have added Attendees to your Group Activity Session, you will need to fill in the Group Activity Attendee Form.  Click on Adding Clients to a Group Activity to review how to add Attendees to your Session.

Instructions and Definitions

Click on 'Expand all rows' to display the Group Activity Attendee Forms for each Attendee in the table.

Please note: An Attendee is the same as a Named Client.

Below each Attendee name, you will see a pink Group Activity Attendee Form. This means the Form is incomplete. Each Form must be filled in to complete the Session.

Each Attendee Form has mandatory Green fields that need to be filled in. Yellow fields are mandatory by IRCC 

Service Funded By: Click on Select a Funder to select the Funder for the Group Activity.

Referred By: From the drop-down menu, select the Referral for the Group Activity

Volunteer Time: Did the attendee do any volunteer time.

Status of Service: Did the service end early, is it ongoing or complete (if you are uploading to iCARE, our bulk upload file does not allow to update records if a service is ongoing.)

Client's Pre-intervention Status

  • Current Employment Status: The client's current employment status.
  • Educational Status: The client's educational status.
  • Current Occupation: See the list of NOC codes.
  • Intended Occupation: See the list of NOC codes

Depending on the type of intervention you chose in the Group Activity form (this will show different options)

Click on the intervention type.

Complete this section for Referral interventions only

Referal to: Yellow fields are only mandatory if you provided this service

Complete this section for Short-term interventions only

Yellow fields are only mandatory if you provided this service

What topics were covered: Click Edit, Select each topic that was covered.

Was the client referred to any specific programs? Type the name of the program(s).

 Were any resources or information provided? Type in the resources or information that was provided.

Short term

Complete this section for Long-term interventions only

Yellow fields are only mandatory if you provided this service

 Mentoring

Average hours spent per week in contact with a mentor that contributed directly to addressing employment needs.  Click Edit, choose the number and click Update

The client met the mentor regularly at: Click Edit, choose the location and select Update.

Work Placement

The size of the employer (thinking of the entire organization): Choose the number and click Update.

Placement is: Is the placement paid or unpaid and click Update.

Client Works: Choose if the client works full or part-time and click Update.

Preparation for licensure/certification

 Profession for which services were received: Please see the List of Regulated Professions. Click Edit, to choose from the list and click Update.

Long term

Computer Skills

  • Does the client possess any computer skills:  Click on the [+] sign to add skills
    • Click Update after each addition.
  • Other applications please specify: Type any other computer skills the client has.

Please choose the programs the client has attended, both past and present,
Click on the [+] sign to add programs

  • Job Search Group Activity
  • Enhanced Language Training
  • Occupational Specific Training
  • Programs Related to Client's Position

The rest of the Form is for 'Client Support Services Received'. Click on Client Support Services to review this section.

It is recommended for ICARE services that each client form be filled in, what can be done in OCMS is to complete one form and copy this form to all other clients.

To copy this form to all attendees

click 'Save' once the Group attendee form is completed and click on copy

All Group Activity Attendee Forms will now be copied to all the attendees and you can make changes to any specific attendee at any time. The forms will appear as Green or Complete.

The Attendee list can be exported to Excel by clicking on 'Export Participant List' at the bottom of the Attendee table.  The Excel table will display the Client ID, Client Name, Attended (checked or unchecked) and Updated (date).

   

Session Notes and Supporting Documents can be added/attached at the bottom of the Form.  

 

Click 'Save Session' when you are done.
