Employment Service Form (2020)

Learn how to add an Employment Form and the six related intervention types.

After you have created a Session form and added a Client you will be able to create an Employment Form.

Instructions and Definitions

An Employment Form collects the information captured during a meeting between a program worker and a client. Services include referrals and short or long term interventions including employment-related task and referral services. This Service form can be added to any client that receives an employment service (in person, by phone,  by email or online).

You need to complete a Session form. Once you have completed the Session form you will see an Information & Orientation form connected to your client (by default). You can Delete this form as it is not needed at this time.

One client or multiple clients (family session) can be included on a Session form. For example, if you work with two family members in the same 35 minute time frame, both are separate "clients" but should be added to the same Session form and a separate Service form is filled out for each one.

Adding an Employment Service Form

Click the Add Form link beside Form Name. Choose Employment Form 2020 and click Update to save it. 

The first time it is displayed it will be in pink to indicate that it is not complete. Click Edit to begin entering information.

Form Overview

Follow the instructions below to fill out this form and register it to OCMS. All fields shaded in Green are mandatory, all Fields shaded in yellow are mandatory if you are reporting to ICARE.

You can now make any Service Form ‘private’, even within a Program. If the Form is designated as Private, only that staff person and Agency Coordinators can see the information.

Tasks and Referrals: This section will be completed at the end of the Employment form if you are creating tasks and/or referrals for the client.


Admin Fields

  • Program: Filled in automatically.
  • Agency: Filled in automatically.
  • Name: Filled in automatically with clients’ name and client number.
  • Session / Form: Filled in automatically.
  • Service End Date: The date the service is expected to end. 
  • Agency Program: Select the program from the drop down menu.
  • Service Funded By: Click Select a Funder and choose the funder this service is covered under.
  • Referred By: Using the drop-down menu, choose how the client was referred to your agency.
  • iCare Program Type: Using the drop-down menu choose the program type you are funded for.
    • If your contribution agreement has Program types then chose the correct one for your service.
    • If your contribution agreement does not have Program types then choose Standard service

Client's Status

  • Employment Status: The client's current employment status.
  • Occupation in Canada: See the list of NOC codes.
  • Intended Employment Status: The client's employment status.
  • Intended Occupation: See the list of NOC codes.

Status of Service: Did the service end early, is it ongoing or complete - We recommend when uploading to iCARE only add completed services

If you upload ongoing services to iCARE, then you need to close all these services directly in iCARE. The iCARE bulk upload does not allow editing of ongoing records that have been already uploaded. (The bulk uploads were created by iCARE).

Target Group

  • Select all that apply by clicking on edit beside the target group(s) and answering 'Yes'.

Intervention Type

As you select the Intervention Type, the fields below will change.  Only ONE intervention type can be chosen.

  • Temporary Work Placement
    • These can include internships (paid or unpaid), practice firms (a form of workplace simulations) and other direct work experience opportunities.

  • Mentoring
    • These can be “speed mentoring” or a traditional link with a professional in the clients’ field of expertise or interest (either in a workplace, informal setting, by phone or online).

  • Preparation for Licensure / Certification
    • Services that are meant to help clients meet the licensing requirements in their field for the purposes of licensure. This is often referred to as bridging and normally is classroom based and focused on a single occupation.

  • Individual Employment Counselling
    • This is direct, one-on-one counselling to clients on pathways to employment.

  • Networking Opportunities
    • Structured activities and events that allow clients to be present in an environment with their professional peers and potential employers to increase their professional networks.

  • Resume Screening and Matching
    • These services involve actively screening resumes of clients to refer those who match an employer’s requirements and/or entering resume into computerized filtering system that
      uploads postings and client’s resumes.

Next Steps

Once you have completed filling in the necessary information, you can add any Tasks or Referrals provided during this session by pressing the Tasks and Referrals button.

Save the Employment Form

Press Save at the top of the page to ensure all information is recorded.
