Employment Group form and Attendee forms order

Due to the complexity of these forms, they must be completed in a specific order to ensure that your reporting will be correct.

  1. Group Form - This form must be completed first to ensure correct settings in the attendee form

  2. Attendee Form for one participant

  3. Copy to all others

  4. All Attendee settings must be completed
  5. Make changes if needed

NOTE: for all attendee setting to work properly on any session type, the main form needs to be completed so the partner can be picked up.

After you have added Attendees to your Group Activity Session, you will need to fill in the Group Activity Attendee Form.  Click on Adding Clients to a Group Activity to review how to add Attendees to your Session.

Instructions and Definitions

Click on 'Expand all rows' to display the Group Activity Attendee Forms for each Attendee in the table.

Please note: An Attendee is the same as a Named Client.

Below each Attendee name, you will see a pink Group Activity Attendee Form. This means the Form is incomplete. Each Form must be filled in to complete the Session.

Each Attendee Form has mandatory Green fields that need to be filled in. Yellow fields are mandatory by IRCC 

Service Funded By: Click on Select a Funder to select the Funder for the Group Activity.

Referred By: From the drop-down menu, select the Referral for the Group Activity

Volunteer Time: Did the attendee do any volunteer time.

Status of Service: Did the service end early, is it ongoing or complete - We recommend when uploading to iCARE only add completed services

If you upload ongoing services to iCARE, then you need to close all these services directly in iCARE. The iCARE bulk upload does not allow editing of ongoing records that have been already uploaded. (The bulk uploads were created by iCARE).

Client's Status

  • Employment Status: The client’s employment status immediately prior to starting the service.
  • Occupation in Canada: The client’s occupation prior to or at the beginning of the service provided. See the list of NOC codes.
  • Intended Employment Status: The client’s planned employment status following their employment services.
  • Intended Occupation: The client’s planned occupation following their employment services. See the list of NOC codes

Find the intervention type you chose in the group activity form to see the questions that should be on your attendee forms.

  • Temporary Work Placement
    • These can include internships (paid or unpaid), practice firms (a form of workplace simulations) and other direct work experience opportunities.

  • Mentoring
    • These can be “speed mentoring” or a traditional link with a professional in the clients’ field of expertise or interest (either in a workplace, informal setting, by phone or online).
  • Preparation for Licensure / Certification
    • Services that are meant to help clients meet the licensing requirements in their field for the purposes of licensure. This is often referred to as bridging and normally is classroom based and focused on a single occupation.

  • Individual Employment Counselling
    • This is direct, one-on-one counselling to clients on pathways to employment.

  • Networking Opportunities
    • Structured activities and events that allow clients to be present in an environment with their professional peers and potential employers to increase their professional networks.

  • Resume Screening and Matching
    • These services involve actively screening resumes of clients to refer those who match an employer’s requirements and/or entering resume into computerized filtering system that uploads postings and client’s resumes.

Status of Intervention/Preparation

We recommend when uploading to iCARE only add completed services.

  • Intervention ended early (i.e. client ended participation)
    • If chosen you will see Reason for client ending participation
      • Found employment
      • To attend school 
      • Personal reason (i.e. parental/family leave, medical, moved, unable to contact)
      • Lack of support services
      • To receive another settlement service
      • Client felt the intervention was not meeting their employment development needs
      • Obtained citizenship o Intervention ended by third-party (i.e. mentor, employer, volunteer)
      • Reason unknown
  • Ongoing
  • Completed

It is recommended for ICARE services that each client form be filled in, what can be done in OCMS is to complete one form and copy this form to all other clients.

To copy this form to all attendees

click 'Save' once the Group attendee form is completed and click on copy 




All Group Activity Attendee Forms will now be copied to all the attendees and you can make changes to any specific attendee at any time. The forms will appear as Green or Complete.

The Attendee list can be exported to Excel by clicking on 'Export Participant List' at the bottom of the Attendee table.  The Excel table will display the Client ID, Client Name, Attended (checked or unchecked) and Updated (date).

Session Notes and Supporting Documents can be added/attached at the bottom of the Form.  

Click 'Save Session' when you are done.
