After you have created a Session form and added a Client you will be able to add and create a Needs Assessment Service form. 

Instructions and Definitions

A Needs Assessment is usually completed the first time you provide a Service to a Client. It includes Settlement questions to assist you in building a Settlement Plan.

A Session form must be created first. Once you have completed the Session form you will be able to add Service forms to the Session.  

An Information & Orientation form is automatically connected to your Client (by default). You can Delete this form as it is not needed at this time and add a Needs Assessment form instead.

Adding a Needs Assessment Service Form

Click the Add Form link beside Form Name. Choose the Needs Assessment Form from the drop down menu and click Update to save it. 

 

The Needs Assessment form is now added. Click Edit to begin entering information. Fill in only the information you received from the Client.

 

Form Overview

Follow the instructions below to fill out the Needs Assessment form and register it to OCMS. All fields shaded in Green are mandatory to save the data to OCMS. Yellow fields are mandatory for iCARE reporting.

You can now make any Service Form ‘private’, even within a Program. If the Form is designated as Private, only that staff person and Agency Coordinators can see the information.

Tasks and Referrals. This section will be done at the end of the Needs Assessment if you select Yes to any of the Needs on the form. The Tasks and Referrals section help you build the Settlement Plan.

OCMS Requirements (the green fields)

  • Service End Date: This date defaults to today’s date, if the Service ended on another date click the drop-down arrow and choose the date.
  • Agency Program: Click on the drop-down arrow and select the Program.
  • Service Funded By: Click on the drop-down arrow and choose the Funder.
  • Referred By: Click on the drop-down arrow and select how the Client found out about your Services.
  • Primary Needs Assessment: Click on the drop-down arrow and select Yes or No.
  • Needs Assessment Type:  Choose one of the following (Pre-Arrival, Settlement Needs, Program Placement or Other)
  • Settlement Plan was completed and shared with client? Click on the drop-down arrow and select Yes or No.

 You can now make any Service Form ‘private’, even within a Program. If the Form is designated as Private, only that staff person and Agency Coordinators can see the information.

Client Questions

Now you will go through the list of possible Client "Needs." At least (2) questions highlighted in yellow should be answered to satisfy iCare requirements.

In each section, only answer the questions you asked the client. To answer the questions click Edit beside the question and select your answer from the drop-down menu. You only need to select the questions that are answered 'Yes', all others can be left at 'Not Set'. The iCare logic is to respond where 'service was provided'.

Any Needs that are identified on the Needs Assessment form will be automatically added to your Tasks and Referrals form.  Continue to the end of each section and make sure you click on the Update links in the section to save your entries.

iCare Requirements 

You must answer one mandatory question plus an additional question from the yellow fields to satisfy iCARE requirements.

 Settlement

  1. Does the client require more information to settle in Canada?
  2. Does the client require more information about community and government services?
  3. Does the client require more information to be able to work in Canada?
  4. Does the client require more information on education?
  5. Does the client wish to expand their social networks?
  6. Does the client wish to expand their professional networks?
  7. Does the client require access to local community services?
  8. Is the client interested in being involved in the community?

The following question is the mandatory requirement for iCARE clients and must be answered:

  • Does the client intend to become a Canadian Citizen?

Language Assessment, Training & Skills Development

  1. If the client is interested in improving language skills, specify purpose
  2. If the client is interested in improving other skills, specify purpose

Employment

If the client is interested in finding employment, specify timeframe?


If this question is answered then the following (3) questions must be answered:

  1. Does the client have a minimum of one year's work experience?
  2. Does the client intend to work in an occupation that corresponds to an NOC skill level? Please specify.
  3. Does the client intend to obtain credential recognition or obtain a license to work in Canada?

Support Services Received

Record any support services that your agency provided so you could complete the service with your client. 

Save the Needs Assessment Form

Press Save at the top of the page to ensure all information is recorded. 

Next Steps

Once you have completed filling in the necessary information, you can add any Tasks or Referrals provided during this session by pressing the Tasks and Referrals button.
