Learn about adding Tasks and Referrals to Service forms to build a Settlement Plan.

Start Here

Click the button: Tasks & Referrals at the top right of every Service form to access the Tasks and Referral page.

 

Instructions and Definitions

Tasks & Referrals are used to build and support your Client's Settlement Plan. Once you have filled in the green fields for Agency ProgramService Funded by and Referred by and selected at least one topic or Need on the Service form, you will be able to access the Tasks and Referrals page.

Form Overview

Admin Fields

  • Settlement Plan: This section is filled in once Tasks and/or Referrals have been created.
  • Plan Name: You can edit the name and date(green field).  The Client's name is automatically filled in with the current date.
  • Managed By: You can set Settlement Plans as Agency or Client Managed.
  • Plan Items: Items will appear on the list as they are added in the sections below.

 

Creating Plan Item: Referral

  • Item Type: Select Referral or Task from the drop down menu.
  • Title: Type in a short description related to the objective of the Referral.
  • Details: Type in comments or details about this Referral. If you use the Notify Client feature this information will be sent to the Client email address listed on the Client Profile.
  • Referral Basis:  Any Needs or Topics selected on the Service forms page will appear on this list.
  • Due Date: Choose the date the Client should complete this Referral by.
  • Referral Type: Referring to an Internal program or External agency.
  • Funder: If this Referral is covered by a Funder, click on Select to choose the Funder.
  • Partner: Select which External agency or Internal program the Client is being referred to.
    • Approved staff can edit this list to add new agencies and programs (including an "unknown" for ad hoc Referrals).
  • Notify Client by Email: Yes or No. (This is greyed out if there is no email address in the Client Profile)
    • If Yes, Clients will get an email with details about this Referral including addresses and due date.
  • Notify Me: Yes or No
    • If Yes, the worker will also receive emails about this Referral.

Creating Plan Item: Tasks

  • Title: Type in a short description related to the objective of the Task.
  • Details: Add comments or details about this Task. If you use the Notify Client feature this information will be sent to the Client email address listed on the Client Profile.
  • Referral Basis:  Only Needs or Topics selected on the Service forms page will appear on this list.
  • Due Date: Choose the date the Client should complete this Task by.
  • Notify Client by Email: Yes or No. (This is greyed out if there is no email address in the Client Profile)
    • If Yes, Clients will get an email with details about this Task including the due date.
  • Notify Me: Yes or No.
    • If Yes, the worker will also receive emails about this Task.

You can create as many Tasks and/or Referrals as needed for the Client.  Once you have saved your Task and/or Referral, you can create a new one on the same page.  When you are done, click on the orange X in the top right corner to exit this page.

Save the Task or Referral Item

Press the Save Item button and add another Task or Referral or scroll to the top and close this item by clicking on the orange X button in the top right corner.

This returns you to the Service form where you will Save and Close the form.

Next Steps

Once you have added one or more Task & Referral items you have created a Settlement Plan. You can find your client's Settlement Plans in their Client Dashboard.
